GENERAL INSTRUCTION FOR ONLINE FORM

1. The information should be filled in CAPITAL letters (in English).

2. The date of birth, qualifications and experience must be supported by photocopies of the certificates. The original certificates should not be attached.

3. The nominees of the institution must send their individual forms duly filled in. Photocopies of the individual application form may be used, if needed.

4. Application for institutional membership must be supported by a bonafide certificate signed by the authorized signatory authority.

5. The fees are subject to review and change from time to time.

6. Money Orders, Indian Postal Orders, cheques or cash will not be accepted for payment of membership/renewal fees.

7. On the BACK SIDE LEFT HAND CORNER of the bank-draft the following details are to be written:
(a) Membership Number (in case of old Members).
(b) Name of the Member.
(c) Address

8. Demand Draft should be issued, payable at Indore, in favour of "All India Association of Computer Centres & IT Professionals" and should be sent directly to

9. In case of Institutional Membership attached authority letter/copy of resolution duly certified alongwith audit reports for last 3 years.

10. Attached 4 recent coloured passport size photographs.
11. Attach photocopy of residential proof i.e. ration card, electricity-telephone bill, voters ID, driving license etc.

12. The Association has sole right to cancel the membership at any time if member found under breach of terms and condition or involved in offensive criminal acts or any kind of anti-social or anti-national activities.
13. All the members will be provided with Membership Certificate, Identity Card, Brotures, Stickers and invitation for grand programme.

14. Subject to Indore Jurisdiction.

I,AFFIRM that as an Association Member, I shall abide by the Code of Ethics of the Association and I FURTHER UNDERTAKE that I shall uphold the fair name of the AACCIP by maintaining high standards of integrity and professionalism. I am aware that my breach of the Code of Ethics may lead to disciplinary action against me under the Byelaws and rules of the AACCIP. I hereby CONFIRM that I shall be bounded by any decision taken by the Association on such matters.
